If you also want to take advantage of this scheme with a free treatment of 5 lakhs, then for this you have to check eligibility first. In such a situation, first of all, go to the official portal https://pmjay.gov.in/ and click on ‘Mi Eligible’ option here.
Step 2
Then you have to enter the OTP received on your registered mobile number. Now you will see that two options will be coming in front of you, in the first you choose your state.
Step 3
Then in the second option, you have to search by your mobile number and ration card number. After this, you will able to know whether your Ayushman card will be able to be made or not.

Eligible people will have to visit their nearest public service center to get an Ayushman card made. Here you have to give your Aadhar card, residence proof, ration card, and mobile number to the concerned authority.
Step 2
Then the concerned officials check your documents and your eligibility. If everything is found to be correct, you are given an Ayushman card within about 15 days.
